This service is for families, carers and participants who feel overwhelmed by paperwork, appointments, reports, service providers, decisions or upcoming meetings.


It may be helpful if:

  • your reports or letters are old, scattered or hard to understand
  • you have an upcoming meeting, review or planning conversation
  • you feel like you are constantly repeating your story
  • you are unsure what evidence may be missing
  • you need help explaining daily support needs clearly
  • your family is under pressure and needs a practical plan
  • you want someone to help you organise what to do next


This service is also suitable for providers and referrers who are working with families who need practical help preparing, organising information and understanding evidence gaps.

We help you look at what is happening now, what documents you already have, what daily challenges need to be explained more clearly, and what may still need to be followed up.

How it works

Step 1: Book or refer


You can book directly through our website, or a provider/referrer can submit an enquiry on your behalf.


Step 2: Complete the intake


We collect basic information about your family’s current situation, upcoming deadlines, current supports, concerns and documents.


Step 3: Attend the consultation


We sit with you and talk through what is happening. We look at your current situation, family stressors, support needs, documents, reports and any upcoming meetings or planning conversations.


Step 4: Review document currency


We check the age and currency of the documents you already have. This may include reports, letters, assessments, provider notes, school information, therapy reports or other supporting documents.


We look at whether the information appears current, clear and useful for planning purposes.


Step 5: Identify what may be missing


We discuss what information may still be needed. This may include missing reports, outdated information, unclear recommendations, lack of detail about daily life impacts, or missing statements from providers or professionals.


Step 6: Contact providers, where agreed


With your consent, we can prepare and send letters or requests to relevant service providers or professionals explaining what information appears to be missing and what may be helpful for planning purposes.


This may help providers understand what details need to be included, such as current support needs, daily life impacts, functional changes, risks, frequency of support, or updated recommendations.


Step 7: Create your plan


We prepare your written Family Evidence Gap & Action Plan, including what you currently have, what may be missing, who may need to provide further information, and what actions to take next.


Step 8: Follow up if needed


Families can choose a follow-up session or ongoing family clarity support if they would like help staying organised and working through the next steps.

We help families understand what information they have, what may be missing, and what may help strengthen their evidence for planning conversations.


We can:

  • help organise documents and information

  • help prepare questions for providers or professionals
  • send letters to providers, with consent, outlining what information may be missing
  • help families create a clear action plan
  • support families to feel more prepared


We do not:

  • upload evidence to any funding body
  • submit applications or documents to funding bodies on your behalf
  • guarantee funding
  • guarantee approvals
  • guarantee plan changes
  • guarantee service outcomes
  • provide legal advice
  • provide clinical advice or diagnosis
  • replace professional assessments or reports
  • tell professionals what they must recommend
  • make decisions on behalf of any external agency or organisation


Our role is to help you understand what may be missing and what steps may help ensure your evidence is clearer, more current and more useful for planning purposes.

Providers and referrers can refer families through our website.


This service may be suitable where a family:

  • feels overwhelmed by documents or meetings
  • has scattered or outdated reports
  • is unsure what evidence may be missing
  • has an upcoming review, meeting or planning conversation
  • needs help organising daily life impacts and support needs
  • would benefit from a clear written action plan


When submitting a referral, please include:

  • participant name
  • family/contact person
  • best contact details
  • reason for referral
  • upcoming meetings or deadlines
  • known evidence gaps or concerns
  • any urgent risks or escalation concerns


Please write “Family Evidence Planning” or “Family Planning & Evidence Consultation” in the referral notes.

 No. This is an evidence planning and organisation service. We do not submit documents to funding bodies or guarantee funding outcomes. 


No. We do not upload evidence to funding bodies. We help you understand what information you have, what may be missing, and what actions you may need to take. We do offer separate Support Coordination Services for this. 


 No. We do not guarantee funding, approvals, plan changes, service outcomes or decisions made by any external body. 


 You can bring reports, letters, assessments, provider notes, therapy reports, school information, medical letters, previous plans, meeting notes, emails or anything else you think may be relevant. 


We can help identify which documents may be outdated and discuss what updated information may be useful for planning purposes. 


 With your consent, we can prepare and send letters to providers or professionals outlining what information appears to be missing and what details may be helpful for planning purposes. 


 No. We do not replace clinical reports, assessments or professional recommendations. Those must be completed by appropriately qualified professionals. 


 You receive a written Family Evidence Gap & Action Plan, which outlines your current situation, evidence summary, possible gaps, priority actions and next steps. 


 Yes. Providers can refer families through our website using the referral or contact form.
