Step 1: Book or refer
You can book directly through our website, or a provider/referrer can submit an enquiry on your behalf.
Step 2: Complete the intake
We collect basic information about your family’s current situation, upcoming deadlines, current supports, concerns and documents.
Step 3: Attend the consultation
We sit with you and talk through what is happening. We look at your current situation, family stressors, support needs, documents, reports and any upcoming meetings or planning conversations.
Step 4: Review document currency
We check the age and currency of the documents you already have. This may include reports, letters, assessments, provider notes, school information, therapy reports or other supporting documents.
We look at whether the information appears current, clear and useful for planning purposes.
Step 5: Identify what may be missing
We discuss what information may still be needed. This may include missing reports, outdated information, unclear recommendations, lack of detail about daily life impacts, or missing statements from providers or professionals.
Step 6: Contact providers, where agreed
With your consent, we can prepare and send letters or requests to relevant service providers or professionals explaining what information appears to be missing and what may be helpful for planning purposes.
This may help providers understand what details need to be included, such as current support needs, daily life impacts, functional changes, risks, frequency of support, or updated recommendations.
Step 7: Create your plan
We prepare your written Family Evidence Gap & Action Plan, including what you currently have, what may be missing, who may need to provide further information, and what actions to take next.
Step 8: Follow up if needed
Families can choose a follow-up session or ongoing family clarity support if they would like help staying organised and working through the next steps.
